1. Notice of Privacy Practices (HIPAA)

We are committed to protecting your health information in compliance with the Health Insurance Portability and Accountability Act (HIPAA).

Your protected health information (PHI) will only be used or disclosed as permitted by law.
You have the right to request copies of your records, request amendments, and ask for restrictions on certain uses or disclosures.
You may also request a list of disclosures of your PHI.
If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services (HHS).

2. Your Right to a Good Faith Estimate (No Surprises Act)

Under Section 2799B-6 of the Public Health Service Act, you have the right to receive a Good Faith Estimate of expected charges if you are uninsured or self-pay.

You may request a Good Faith Estimate for the total expected cost of any non-emergency items or services.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Please save a copy of your Good Faith Estimate for your records.

3. Non-Discrimination & Accessibility Statement (ACA §1557)

Fresh Start Marriage & Family Therapy, PLLC complies with applicable Federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, religion, sexual orientation, gender identity, or sex.

We provide:

Free language assistance services to people whose primary language is not English.
Accessible services for individuals with disabilities.

4. Telehealth Services Disclosure

Fresh Start MFT offers services via HIPAA-compliant telehealth platforms.

Telehealth sessions are subject to the same privacy protections as in-person therapy.
Limitations: Telehealth may not be appropriate for all conditions. Emergency or crisis situations may require in-person care.
If you are experiencing an emergency during a telehealth session, please call 911 or go to your nearest emergency department.

By engaging in telehealth services, you acknowledge and consent to these limitations.

5. Emergency Disclaimer

Fresh Start Marriage & Family Therapy, PLLC is not a crisis center.

If you are experiencing an emergency, call 911 immediately.
If you are in emotional distress, you may also call or text 988, the Suicide & Crisis Lifeline, for immediate support.
